If you fall asleep to rain, white noise, or a podcast with earbuds in, you have probably wondered whether you are slowly wrecking your ears. Most articles on this answer with a vague warning about "hearing fatigue" and then sell you a headband. That warning is mostly wrong, and it distracts from the risks that are real.
The short version: at sleep-appropriate volume, hearing damage is not the problem. The math on safe listening exposure says so clearly. The real costs of nightly earbud use are mechanical and microbial, blocking the ear canal for eight hours at a time, plus a handful of practical safety issues nobody mentions. Is it bad to sleep with earbuds every night?
Sleeping with earbuds every night is not dangerous for most adults at low volume, but it is not risk-free either. The main problems are ear-canal occlusion for eight hours at a stretch, which raises earwax impaction and outer ear infection risk, and physical pressure if you side-sleep. Hearing loss is unlikely below about 70 decibels.
That framing matters because the two risk categories have different fixes. If your worry is hearing, the answer is a volume setting. If your worry is your ear canal, volume is irrelevant and the answer is exposure time, hygiene, and fit.
It also matters who you are. Narrow ear canals, a history of swimmer's ear, eczema, or heavy earwax all raise your odds of trouble. If you have never had an ear problem, nightly earbuds at 50 decibels are a small risk. If you get blocked ears twice a year, they are a bad idea.
Can sleeping with earbuds damage your hearing?
Sleeping with earbuds is very unlikely to damage your hearing at normal sleep volumes. The World Health Organization and ITU safe-listening standard allows adults 80 decibels for 40 hours per week. At the 50 to 60 decibels appropriate for sleep sounds, your weekly allowance runs to thousands of hours, far more than a week contains.
Here is the arithmetic people skip. Noise exposure limits trade level against time on a fixed exchange: drop the volume by 3 decibels and you double the time you can safely listen. Starting from the WHO-ITU reference of 80 decibels for 40 hours a week, dropping to 60 decibels is a 20 decibel reduction, which multiplies the allowance by roughly 100. That is about 4,000 hours a week of safe listening. A week contains 168 hours.
Even eight hours a night, seven nights a week, only adds up to 56 hours of listening. To push that into genuine hearing-risk territory you would need to sustain roughly 78 decibels all night, which is busy-restaurant loud playing directly against your eardrum. If your audio is quiet enough to sleep through, it is quiet enough to be safe.
There is one real exception. People who crank the volume to drown out a snoring partner, a thin wall, or street noise can drift well past 70 decibels without realising it, because earbuds put the driver millimetres from the eardrum and your loudness reference is gone. The headline risk of nightly earbud use is not your cochlea, it is your ear canal. Volume is the wrong dial to be worrying about if you already sleep at a level you can think past.
What actually happens to your ears overnight?
An earbud seals the ear canal, trapping heat and moisture and interrupting the canal's self-cleaning process. Skin in the ear canal migrates outward at roughly the rate fingernails grow, carrying wax with it. A silicone tip blocks that conveyor and can press wax inward, which is why earbud users show higher rates of cerumen impaction.
Two conditions account for nearly all earbud-related ear complaints, and both are consequences of occlusion rather than sound.
Cerumen impaction. The 2017 AAO-HNS clinical practice guideline puts baseline prevalence at about 1 in 10 children, 1 in 20 adults, and more than a third of older adults. It explicitly names hearing aid, earplug, and earbud users as a higher-risk group, and advises hearing aid wearers to leave the devices out for around eight hours a day to let the canal recover. An eight-hour nightly earbud habit is, functionally, the pattern that guidance warns about. Symptoms are muffled hearing, fullness, ringing, or a dull ache, and clearing it needs a clinician, not a cotton bud.
Acute otitis externa. Better known as swimmer's ear. Earwax is slightly acidic and mildly antibacterial, so a canal that clears itself resists infection well. Seal it under a warm, humid earbud tip for eight hours and you get a much friendlier environment for bacteria. The CDC's burden estimate, drawn from 2003 to 2007 data, found 2.4 million healthcare visits a year for acute otitis externa in the United States, affecting about 1 in 123 people annually at a direct cost near 500 million dollars. Those numbers predate mass earbud use and are mostly water-related, so treat them as scale rather than blame. The mechanism, a warm wet canal that cannot dry out, is the same one earbuds create.
The third issue is purely mechanical. Side sleepers press a hard plastic housing into the canal wall with their full head weight for hours. In rare but documented cases that focal pressure restricts blood flow enough to injure the canal skin. Far more often it just produces a sore ear you blame on the pillow.
Do earbuds make your sleep worse?
Overnight audio can help or hurt depending on what it replaces. Continuous sound genuinely reduces sleep disruption when it masks an unpredictable environment, but the evidence that it improves sleep in an already quiet room is weak. Earbuds add their own costs: physical discomfort, an abrupt cutoff when the battery dies, and masked alarms.
The strongest evidence for sound as a sleep aid is about masking, not sound itself. A 2005 study in Sleep Medicine by Stanchina and colleagues played recorded intensive care unit noise to healthy sleepers. Their arousal index went from 13.3 per hour on a quiet baseline night to 48.4 per hour under ICU noise. Adding mixed-frequency white noise brought it back down to 15.7 per hour, essentially baseline. The critical finding was that arousals tracked the change from background level to peak, around 17.6 decibels in both conditions, rather than the peak volume itself. Sound helps by raising the floor so spikes stand out less.
What that does not establish is that overnight audio helps in a room that is already quiet. Riedy and colleagues reviewed 38 studies for Sleep Medicine Reviews and rated the evidence that continuous noise improves sleep as very low quality. Some polysomnography work also suggests continuous overnight sound can trim REM. Masking a noisy environment is well supported; running sound all night in silence is not.
Earbuds then add practical problems on top:
- Alarm masking. Active noise cancellation is very good at removing exactly the sounds you might need, including your alarm, a smoke detector, or someone calling you.
- The silence cliff. When the battery dies at 3am, the sound stops abruptly. A sudden drop in the noise floor wakes people the same way a sudden noise does, which is the same mechanism the Stanchina study identified.
- Position lock. If lying on one side hurts, you stop turning over freely, and restricted movement is its own source of fragmented sleep.

Are wireless earbuds safer than wired ones for sleeping?
Wireless earbuds are safer than wired for sleep in one important respect: no cord to wrap around your neck. They introduce different problems instead, including small parts that get lost in bedding, lithium batteries next to your head, and a hard shell that presses into the canal when you roll over. Neither type is designed for eight-hour wear.

To reduce risk, keep volume below about 60 decibels, use a sleep timer that fades audio out within 30 to 60 minutes rather than running all night, take at least one earbud-free night per week, and clean the tips regularly. The timer matters most: it cuts canal occlusion from eight hours to under one.
The full practical routine:

- Set volume before you are tired. Pick the level while alert, when you can still judge it. Tired ears reliably ask for more.
- Give your ears a night off. The earwax guideline applies the same logic to hearing aids: one or two bare nights a week lets the canal dry and clear.
